Prescribed forms are available for submission of LTC claim and LTC advance.
Important to Note: Submit the LTC claim within 30 days of the due date where LTC advance is drawn and within 60 days of the due date where no advance is drawn, as prescribed under Rule 292 of GFR 2017.
